This role is an essential member of the Treatment Team, coordinating behavioral health and community-based services to help youth stabilize, achieve treatment goals, maintain placement, and work toward permanency with their families.As a Case Manager, you will collaborate closely with foster parents, clinicians, the Idaho Department of Health & Welfare, and birth families to develop individualized treatment plans and ensure consistent progress.
