The role includes case finding, health teaching, health counseling, and the provision of supportive and restorative care under the direction of a registered professional nurse.Primary ResponsibilitiesProvide symptom assessment, management, and supportive counseling to consumersAdminister medications and document activities in accordance with agency medication administration protocols; maintain copies of provider prescriptionsAssist in the formulation of nursing diagnoses and treatment plan recommendations, including short- and long-term goals, as part of an interdisciplinary teamAssess consumers' health conditions daily and communicate changes in needs and plans of care to team membersImplement physician orders accurately, both directly and indirectlyReport consumer needs, problems, and health observations to the Registered Nurse and/or physicianObserve and document consumer behavior and responses to treatment, rehabilitation programs, support services, and medications in accordance with agency and audit standardsDocument significant behavioral responses and assist in crisis and emergency situations to protect consumers, staff, and othersPerform weekly or assigned inventories of medical supplies and maintain accurate counts of controlled substancesMaintain medication records on a monthly or assigned basisParticipate in fire drills, headcounts, fire-watch, and heat safety activities; assist with accounting for residents/consumers and missing person procedures as requiredMaintain accurate and timely case records, including progress notes, service plan reviews, assessments, medical records, and daily logbook entriesProvide crisis assessment and interventionAssist consumers with basic daily living activities and support social activities and relationshipsProvide or arrange emergency medical careConduct or assist with educational discussions on medication effects, coping skills, assertiveness training, sexuality, and related topicsProvide health-related referrals and community resource informationParticipate in staff development activitiesEnsure the medication room is clean, organized, and maintainedPerform other related duties as assigned#ICLRN
