Absolut Care of Aurora Park. Abslut Care of Aurora Park is a three hundred twenty (320) bed facility located in the charming village of East Aurora, New York. Convenient to the Buffalo and Rochester metropolitan areas, our facility specializes in short-term rehabilitation, secure dementia care and long-term skilled nursing care provided in a family oriented environment. Our Physical, Speech and Occupational Therapists are full-time employees at Absolut Care of Aurora Park. They are able to focus solely on the successful treatment and rehabilitation of our residents. Our "Care Partnership Program" aligns our Rehab staff closely with a resident's personal physician/surgeon, so that the prescribed protocols may be implemented and an individualized treatment plan can be developed to meet each resident's needs. Therapy services are offered 6 days per week, when prescribed, allowing residents the opportunity to progress more rapidly toward their return home. Our state of the art rehabilitation unit offers VitalStim therapy to treat Dysphagia and Anodyne therapy to treat complex wounds and restore nerve function. At Absolut Care of Aurora Park, we also specialize in caring for residents and their family as they move through the last stages of their life. We offer specialty training for staff to increase their sensitivity to the difficulties that residents and family members experience during this time. Our residents are part of our family and we never lose sight of the important role they play in their own family.Our Dialysis RN will be responsible for coordinating and providing nursing care, training and education. This RN will work collaboratively with team members and participants in program development.Primary Clinical Responsibilities (Including but not limited to):Responsible for execution of dialysis programCollects, analyzes and records relevant patient health data accuratelyAnalyzes assessment data to identify patient problemsDevelops a plan of care that describes interventions to attain expected outcomes Implements and records interventions to attain expected outcomes in EMREvaluates patient progress toward attainment of identified goalsAdheres to approved, research-based practice standardsUtilizes equipment and materials in a safe and cost-effective mannerPatient Population: Adult and GeriatricEMR: Crown WebDress Code: Dialyze Direct will provide black scrub tops.
