Registered Nurse Home HealthSchedule: 8 hoursAssignment Details:Contract Length: 13 weeksGuaranteed Hours: 40Requirements:Current state license as a Registered NurseHomecare Homebase Software ExperienceOASIS ExperienceCHPN certificationProof of current CPR certificationCurrent driver's licenseMust have reliable transportation and insurance1-2 years experience as a Registered Nurse in a clinical care setting2+ years Home health experienceRoles & Responsibilities:Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.Implement, develop, document, and evaluate the Plan of Care (POC).Educate patients and/or their families regarding care, medications, and safety.Supervise and provide clinical direction to home health aides to ensure quality service.Maintain up-to-date patient records and documentation.Provide effective communication with patients, families, and healthcare teams.Compliance / Notes:Must maintain confidentiality.Ability to work flexible schedule and travel locally.Must meet mandatory continuing education requirements.Client Details:City: FlintState: MITLC Nursing Associates, Inc. is an equal-opportunity employer and fully complies with all applicable federal, state, and local laws.
