Both are full-time, Monday through Friday day shift positions.Position 1 - RN Navigator, Structural Heart. The Structural Heart RN Navigator works independently to plan, implement, and evaluate nursing care in collaboration with the Structural Heart Team. This role leads care coordination for TAVR, WATCHMAN, and other structural heart patients, guiding them from initial referral through post-procedure follow-up.Position 2 - RN Navigator, Cardiothoracic Surgery. The Cardiothoracic SurgeryRN Navigator works directly with cardiac surgeons and PAs to manage the full continuum of care for cardiothoracic surgery patients. This role oversees the referral process, pre-operative workup, patient and family education, and post-discharge coordination, including collaboration with home health agencies.Benefits & CompensationHourly pay range between $34-$45/hrComprehensive medical, dental, and vision coverageAccident, critical illness, and wellness perks401(k) with retirement planning supportLife insurance, AD&D, and disability coverageFlexible spending accounts and preferred legal servicesEmployee assistance programsTuition reimbursement for continued educationPaid time off to recharge and maintain work-life balanceOptional supplemental benefits to fit your lifestyle QualificationsActive Registered Nurse (RN) license in the State of Florida requiredMinimum of 2 years of cardiac care experience requiredBSN preferredBasic Life Support (BLS) certification from the American Red Cross or American Heart AssociationStrong communication, critical thinking, and problem-solving skillsAbility to work independently and maintain flexibility in a fast-paced environmentIntermediate computer proficiency: Microsoft Word, Excel, Outlook, and Windows Key ResponsibilitiesReferral Coordination & Patient AccessReceive and triage incoming referrals from physicians and hospitals, ensuring timely access to the programCommunicate acceptance plans clearly to referring providers and keep patients and families informed throughout the processCoordinate outreach to current and prospective referring physicians Patient Assessment & Care PlanningPerform thorough clinical assessments, including psychosocial evaluation, surgical risk scoring, frailty and mobility assessment, and KCCQSchedule and manage appropriate diagnostic workup per protocol (e.g., Cardiac Cath, TTE, TEE, vascular studies)Develop and coordinate pre-admission, inpatient, and post-discharge care plans in collaboration with the multidisciplinary team Patient & Family EducationCreate and implement educational materials tailored to patient and family learning needsProvide consistent education on the referral process, procedure steps, and post-operative expectationsAssist with inpatient education alongside APRNs and program staff as applicable Quality, Registry & Data OversightMaintain accurate program databases and comply with all documentation protocolsStructural Heart: Participate in TVT and WATCHMAN registry submissions; identify and resolve data quality issuesCT Surgery: Maintain STS data integrity; facilitate solutions to quality improvement issuesFacilitate quarterly Quality and Finance meetings with organizational leadership Research & Professional DevelopmentStay current on relevant literature and evidence-based standards; share knowledge with the teamParticipate in and support research projects; identify potential research topicsMaintain and regularly review clinical protocols to ensure they reflect current evidenceAttend conferences and educational forums to uphold up-to-date practice standardsMaintain all required compliance training and pursue relevant professional certifications Staff Education & LeadershipServe as a clinical resource for all nursing units caring for this patient populationOversee internal staff education and competency maintenanceCT Surgery: Responsible for orienting new team members to the CT Surgery programDemonstrate professional leadership and support a collaborative, team-based practice environment Background Screening Requirement (Florida)This position requires background screening through the Florida Care Provider Background Screening Clearinghouse in accordance with Florida law (Section 435.12).
