Job Duties: Provide 1 on 1 personal care to patients in their home using patient-specific care plan developed by RN Case Manager, to include: bathing and personal hygiene tasks, ambulation and transfer assistance, vital signs, and other ADL activities Works closely with the patient/family/caregivers and other members of the interdisciplinary team Completes point-of care documentation Ability to meet productivity expectations of the agency Requirements of Qualified Candidates: Current Oregon CNA license Minimum of 1 year full-time direct patient care experience Prior use of Electronic Medical Records (EMR), preferred. Ability to maintain composure in addressing stressful and/or challenging situations. Current & valid OR Driver's license Current & valid car insurance CPR certified. Dependable car Ability to function as a member of the Interdisciplinary team communicating at least weekly with the team. Must be able to complete and pass health screenings. Must be able to pass a criminal background check.
