Supportive Services for Veteran Families Case Manager
Job Summary:
We are seeking a dedicated Case Manager to join our team. The Case Manager will be instrumental in providing comprehensive support to individuals in need, ensuring their well-being and facilitating access to essential services.
Abilities Required:
-Must be proficient with computers
-Must be able to communicate clearly and professionally both in the spoken and written word
-Strong ability to work as a team as independently
-Must possess organizational, planning, information gathering, information monitoring, problem analysis and problem-solving skills
-Must possess judgment and decision-making ability, initiative, confidentiality, and attention to detail and accuracy
-Must have Associates or Bachelors Degree
-Valid NYS Drivers License
Responsibilities:
-Complete pre-screens on all Veterans that seek assistance.
-Conduct community outreach
-Work with other agencies to ensure Veterans in need are being reached.
-Compile data for monthly reports
-Complete and file all applicable reports provide direct client services
-Perform eligibility assessments to determine that Veterans are eligible for SSVF Assistance determined by the CNYVOC, that the Veteran is homeless or at imminent risk.
-Provide holistic and comprehensive case management services to all clients, with a strong focus on SSVF tenants: including intake assessment, benefit assessment, goal setting, long –term case management, tenant education, advocacy, and referrals.
-Assist the Housing Director with the maintenance of the housing binders, tenant leases, and other paperwork
-Assist the Housing Director in monthly housing inspections
-Identify opportunities for collaborative ventures with local apartment complexes, rental agencies, and owners of rental houses to begin the process of finding local affordable housing for Veterans.
-Ensures all applicable regulations and guidelines are followed before requesting approval for assistance.
-Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
-Help all Veterans and their families that are in need.
-Attend community-based meetings when needed.
-Work closely with the VA, DOL and any other government agencies and community agencies as needed.
-Complete SOAR applications for eligible clients
-Input HMIS data

Pay: $15.00 - $20.00 per hour
Expected hours: 35 – 40 per week
