This contract assignment requires serving patients across designated territories, with possible additional travel as needed to ensure full case management. Weekend coverage may be occasionally required to support optimal patient outcomes and meet weekly visit expectations.Schedule & Hours: 36 hours guaranteed for qualifying assignments, with at least 27 points completed per week, based on visit type. Flexibility to accept non-guaranteed hours if preferred, with visit-based scheduling. Assignments may include weekends and travel outside assigned counties. Start and end dates are fixed; requested time off (RTO) must be submitted during application. Qualifications: Current and valid CA Physical Therapist (PT) license. BLS certification. Valid driver's license, auto insurance, and vehicle registration. Up-to-date health screenings: TB (annual), MMR, Varicella, HepB, Tdap (within 10 years), Influenza (annual), and drug screening. Completion of a physical exam. Strong documentation, organization, and communication skills. Responsibilities: Deliver high-quality physical therapy services to patients in their homes, following individualized care plans. Complete thorough documentation, including OASIS assessments such as ReCertification, Routine, Evaluation, Discharge, and related follow-ups (each qualifying for points/hours as defined by visit type). Travel within assigned and occasionally neighboring areas to fulfill caseload requirements. Maintain effective communication with healthcare providers and patients' families.
