Job Title: RN Nurse Care Manager – Hybrid (Brooklyn, NY) Location: Queens, NY 2-4 weeks On-site training in Queens, Office Work Model: Hybrid – field visits + remote work Job Type: FTE/Permanent Languages : Spanish or Cantonese/mandarin or Russian preferredWork Schedule & Structure Hybrid role: Field + Remote Field work: ~6 home visits completed in 1 day (Queens territory) Remote work: Care coordination, documentation, and follow-ups Training location:Queensoffice Orientation: BronxAbout the Role We are seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines limited field work with remote care coordination, offering strong work-life balance while making a meaningful impact in the community.What You’ll Do Conduct in-home assessments to evaluate members’ clinical, functional, and social needs Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams Coordinate care across home care, hospital, rehabilitation, and long-term care settings Complete approximately 6 home visits per week within Brooklyn Monitor patient progress and adjust care plans to ensure optimal outcomes Support safe discharge planning and reduce hospital readmissions Communicate with members, families, caregivers, and providers Maintain accurate and timely documentation in compliance with guidelinesQualifications Active, unrestricted RN license in New York Associate’s degree in nursing required (BSN preferred) Minimum 3 years of RN experience Experience in MLTC, home care, geriatrics, or case management preferred Strong communication, organizational, and critical thinking skills Ability to work independently in a hybrid environment Bilingual skills are a plusWhat We Offer Competitive salary: $100K – $130K per annum Benefits: Dental, Vision, life, PTO, Retirement plan etc. Hybrid schedule Supportive, mission-driven team Opportunity to make a direct impact in patients’ livesApply Now If you’re a compassionate RN looking for flexibility and meaningful community-based work, we encourage you to apply today.
