This position works under the supervision of a Registered Nurse, providing high-quality, person-centered care to residents in a collaborative and supportive environment.Essential Leadership ExpectationsWe are looking for team members who:• Act with integrity, respect, and honesty• Communicate openly and listen thoughtfully• Seek continuous improvement and drive for results• Support teamwork, mentorship, and growth• Think critically, adapt to change, and problem-solve effectively• Build strong relationships across the organization• Engage positively with the community and prioritize service• Demonstrate emotional intelligence and resilienceKey Responsibilities• Provide nursing care within the provisions of the Nurse Practice Act• Administer medications and treatments under RN supervision• Document clinical observations accurately in resident records• Assist in training and orientation of new staff• Support development and implementation of resident care plans• Maintain resident rights, confidentiality, and safety standards• Prepare daily shift reports and communicate resident status changes• Ensure efficient use and supervision of Nursing Assistants• Participate in in-service education and ongoing training• Promote person-centered care and uphold a positive, respectful environmentPhysical Requirements• Up to 75% standing/talking; 50% walking/hearing/near vision• Up to 25% sitting, bending, twisting, reaching• Up to 10% stooping/crouching; lifting up to 60 lbs; pushing wheelchairs• Routine use of nursing-related equipment• Tasks may involve exposure to blood or body fluidsQualifications• Graduate of an accredited nursing program• Current Wisconsin LPN license (required)• Experience in long-term care preferred• Valid WI driver's license and acceptable driving record (if driving required)• Successful completion of background check per Wisconsin Caregiver LawWhy Join Lakeview Health Center?Be part of a compassionate team dedicated to providing exceptional long-term care to our community.
