In this role, you will be the first point of contact for patients, helping them understand how our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services can improve their health outcomes. This is a vital position that directly impacts patient wellness while supporting our clinical teams. You will be responsible for reaching out to patients, determining their eligibility, and clearly explaining the benefits of our programs to gain their consent.Daily Responsibilities:Conducting professional, compassionate outreach to patients via phone to introduce CCM and RPM programsEvaluating patient eligibility and clearly explaining program benefits and expectationsAccurately documenting verbal consent and patient interactions in the EMR, strictly following CMS and HIPAA guidelinesCollaborating with internal care teams to ensure smooth hand-offs and consistent patient supportMaintaining high standards of documentation to ensure accuracy for care planning and billingLong-term Success:Over time, you will become a subject matter expert in our enrollment workflows, contributing to higher program participation rates and improved patient engagement metrics.
