PRNEvery 4th weekend2 holidays/year.Minimum: 16 hours/month weekend, 8 hours/month on week days.General SummaryProvides professional nursing services to individuals in the home through a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on VNA assignment.Duties and ResponsibilitiesEssential Functions:Independently recognizes subtle changes in condition and modifies plan of care accordingly. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. Initiate's emergency action in response to acute risk situations.Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.Evaluates patient care and promotes goal-directed change to achieve patient outcomes. Able to discuss, document and apply Performance Initiatives and outcomes at the VNA level as they relate to the WellSpan Bold Goals.Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients. Initiates interdisciplinary collaboration in evaluation of patient progress. Identifies and facilitates individualized patient needs for transfer/ discharge and includes patient/family in decisions.Begins reading current literature. Express interest in evidence-based research and its application to best practice. Understands clinical relevance and its impact on patient outcomes.Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, including state mandated continuing education credits for licensure renewal. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description.In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLSCommon Expectations:Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.
