This role involves managing acute and chronic medical conditions while collaborating with an interdisciplinary healthcare team to support whole-person care.As a Federally Qualified Health Center (FQHC), Open Door Health Services provides comprehensive primary, urgent, preventive, and select specialty services, along with social support programs for individuals and families—regardless of their ability to pay.
