Right at Home focuses on meeting the unique needs of each individual with compassion and respect.Role DescriptionThis is a full-time on-site role for a Community Outreach Coordinator, located in Whitehall, PA. The Community Outreach Coordinator will develop and maintain relationships with hospitals, assisted living, independent living, and skilled rehab partners to drive home care referrals.The role will also include implementing community outreach initiatives to support program goals. The role involves planning and coordinating events, collaborating with community partners, and representing the organization to enhance awareness and engagement.QualificationsStrong Interpersonal Skills and the ability to foster positive relationshipsDemonstrated experience with developing referral relationships with hospital case managers, social workers in rehab environment, assisted living and independent living community to drive home care referrals. Excellent Communication and Cooperation skills, including verbal and written communicationProficiency in planning and executing Community Outreach programs and initiativesGood organizational and time management skillsAbility to work on-site in Whitehall, PAExperience in a health care setting a must- experience selling to and driving referral relationships.Bachelor’s degree in Communications, Public Relations, Social Work, or a related field is preferred
