Position Overview Join a dynamic clinical team supporting a specialized Pharmacy Advisor program focused on improving outcomes for patients with chronic conditions. This role offers the opportunity to directly impact patient health through education, adherence support, and collaboration with providers using innovative tools and engagement strategies. What You Will Do Provide consultation and education to patients regarding disease states and medication therapies Conduct outbound and inbound calls to support adherence and optimize treatment outcomes Partner with patients to address barriers to medication adherence and overall care Collaborate with prescribers and internal teams to evaluate therapy needs and recommend solutions Utilize advanced systems to identify patients for targeted clinical interventions Apply behavioral modification and motivational interviewing techniques to improve outcomes Maintain accurate clinical documentation and ensure compliance with pharmacy regulations Participate in ongoing program development, process improvements, and training initiatives What We Are Looking For 1 plus year of experience in retail, long term care, hospital, PBM, or pharmacist to patient consultation Experience supporting patients with chronic conditions such as diabetes preferred Strong knowledge of drug utilization review and pharmacy regulations Excellent patient consultation and customer service skills Strong written and verbal communication skills Comfortable working with multiple systems and learning new technologies Highly organized, flexible, and patient focused mindset Work Environment Remote work from home role Fast paced, high volume call center environment Safety sensitive position 469-342-1411 .
