ABOUT THE TEAM The Personal Supports teams provides both at-home and community supports for people with intellectual and developmental disabilities during morning, mid-day, and evening hours. We focus on each person's individual goal, gifts, and areas of interest as we customize schedules to support people within their communities every day. IN THIS ROLE, YOU'LL: Support an energetic individual who is working toward greater independence and confidence at home and in the community. They are developing independent living skills, particularly in the kitchen, while strengthening their ability to follow multi-step instructions, improve safety awareness, and build daily life skills. They enjoy grocery shopping, where they practice planning and budgeting, and participate in community-based social and recreational activities that support interaction and wellness, including regular gym routines. Motivated and open to learning, they continue to grow through consistent, person-centered support that promotes independence and skill development.Implement the person center plan which supports people in achieving the outcomes and goals.Aid the person you will support with personal care, medical, behavioral, or safety needs.Research for community resources and other resources (i.e.: education, spiritual, leisure, transportation and medical).Facilitate successful participation in events, including staff training, community education, technology, and resource development.Collaborate with Team Coordinators and staff to ensure people's transportation, accommodations, etc. for the people you support.
