Responsible for assessment of clients, development and implementation of an Individualized Action Plan. Provides Individual, Group, Family and Crisis Treatment Services as appropriate. Also is responsible for referral to other appropriate services and coordination with other service providers. Performs related tasks as required.MAJOR DUTIES AND RESPONSIBILITIES:Knowledge and demonstration of agency core values in day to day activitiesMeets productivity standard of 100% annuallyMeets documentation timeliness standardsDemonstrates ability to effectively use technology and appropriate use of supportFollows Agency policies and proceduresCompletes yearly update trainingsHandles intake calls and performs initial assessments commensurate with regulatory requirementsDevelops and reviews client individual action plan, objectives and methods consistent with assessment of problemConducts individual, group and family sessionsCoordinates services with other staff and other agenciesImplements established treatment plan and provides assigned professional interventionsContacts collaterals and family members to encourage their participation in client's treatment as well astheir own treatment, if neededServes as resource person/consultant to other service providers in identifying clients for referral and works cooperatively with other providers to assure linkage of clients with appropriate servicesPrepares reports, maintains clinical files, participates in training and provides statistical materialsAssures that advocacy, linking, expediting and related activities are carried out as neededPerforms other duties as assignedSKILLS/COMPETENCIES:Credentialing as a Certified Alcoholism and Substance Abuse Counselor and five (5)years experienceLicensure as a Social Worker and two (2) years experienceCertification as a Rehabilitation Counselor and two (2) years experienceLimited Permit or Licensure as a Mental Health Counselor and two (2) years experienceEDUCATION REQUIREMENTS:Master's Degree preferred commensurate with required licensingEducational qualifications must match regulatory requirementsEXPERIENCE:Experience must match regulatory requirementsPreferred: One (1) year experience in Human Services working with a population of individuals with Serious Mental Illness (SMI) or Substance Use Disorder(SUD)OTHER:Must possess a valid Driver's License with an acceptable driving record.COMPENSATION:$51,000-$66,000/yearly with a $5,000 Sign-on Bonus
