Care ManagerAloma Healthcare, Inc. seeks a talented and experienced Care Manager to join us for a full-time role as an individual contributor. The Care Manager will be responsible for leading patient care teams and ensuring provision of quality healthcare services to all patients. The Care Manager must possess excellent communication, problem-solving, and customer service skills, as well as the ability to remain organized and multitask.ResponsibilitiesLead patient care teams to ensure provision of quality healthcare servicesDevelop team performance plans, goals, and objectivesOversee patient care services, including scheduling, tracking, and evaluatingMonitor patient care processes and adjust as neededMaintain up-to-date records of patient care servicesEnsure compliance with all healthcare laws, regulations, and policiesMonitor patient care teams to ensure quality and compliance with safety protocolsCommunicate proactively with patients, management, staff, and other healthcare professionals to foster positive relationsGenerate KPIs (key performance indicators) and PIP (performance improvement projects) and metrics to improve company base and client level improvementsConduct and oversee quality meetings (QAPI) and conduct compliance operation audits (client and staff files)Attend conferences and marketing events and manage media relations/external communications50% of assignments will involve home care responsibilitiesRequirementsBachelor's degree in related healthcare field5+ years of professional healthcare experienceStrong knowledge of healthcare regulations and lawsExcellent communication, problem-solving, and customer service skillsProven ability to lead teams in a positive, productive mannerEEOC StatementAloma Healthcare, Inc. is an Equal Employment Opportunity/Affirmative Action Employer.
