As an Admission Concierge, you will play a critical role in ensuring a seamless transition for new patients and their families, coordinating the admission process, and providing exceptional customer service. Responsibilities:• Coordinate the admission process for new patients, including gathering necessary information, completing paperwork, and scheduling appointments • Develop and maintain relationships with patients, families, and healthcare providers to ensure a smooth transition to our care • Utilize expertise to identify and resolve issues that may impact the admission process • Communicate effectively with patients, families, and healthcare providers to address concerns and provide updates • Maintain accurate and up-to-date records of patient admissions, including demographic information, medical history, and treatment plans • Collaborate with the hospital and healthcare team to ensure a comprehensive approach to patient care • Participate in quality improvement initiatives to enhance the patient experience and improve outcomes Requirements:• Nursing home Skilled Admission Concierge experience preferred • Ability to work in a fast-paced environment with high attention to detail and excellent organizational skills • Exceptional communication and interpersonal skills • Ability to work effectively with patients, families, and healthcare providers • Familiarity with electronic medical records and medical terminology Why join our team?Anderson Mill Center for Nursing and Healing is a [briefly describe the company culture or mission].
