Department Behavioral Health Coordination Location 200 W Front St Position Type FT Employee Salary Open Date 02/24/2026 Description Case Manager – FUSE Program Starting salary range: $23.7738 - $26.1532 per hour Position Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m. (full time) Monthly on-call rotation scheduled within team Location: Bloomington, IL Position Summary Under general supervision of the Frequent User System Engagement (FUSE) Program Manager, and in coordination with the Director of Behavioral Health Coordination, this position performs professional social service work, both in the office, in the community, and in their natural environments. The FUSE Program serves those individuals, aged 18+ who have intersected most frequently with two or more high-need/high-cost systems of care within McLean County (emergency departments, homeless shelters, justice system), with an emphasis on trauma-informed care, harm reduction, and unconditional positive regard. This position is part of an intensive case management team following a Community Support Team standard of care. The level of acuity of the participants served by this program makes this role difficult for an entry level clinician or for someone with no previous experience with human services. Work requires individuals who can maintain calm, even in stressful situations. Work follows principles of recovery, social service professional standards, and County policies and procedures. Essential Duties and Responsibilities Participate in daily team meetings and weekly supervision Involvement in the ongoing care of program participants, while functioning as a case manager Advocate for participant needs Engages in outreach to potential new participants, as a representative of the FUSE Program Answers, screens, and handles any inquiries by phone or in person Communicates with participants in a professional, friendly, courteous, and welcoming manner Maintain and protect confidentiality Maintains appropriate and necessary documentation Prepares goals and treatment plans for participants Performs related work as required Knowledge, Skills and Abilities Knowledge of mental health programming, case management practices, therapeutic models for severe and chronic mental illness, and available community resources. Understanding of criminal justice system processes, community supervision dynamics, and the intersection of justice involvement, recidivism, homelessness, and behavioral health. Ability to provide compassionate, trauma-informed support to individuals experiencing chronic mental illness and behavioral health crises. Skill in advocacy for participants’ needs with internal leadership, external partners, and community agencies, including law enforcement and service providers. Knowledge of housing, employment, and recovery-oriented supports (preferred). Strong interpersonal, communication (oral and written), conflict resolution, and problem-solving skills. Ability to build and maintain effective working relationships across diverse populations, including justice partners, legal professionals, community providers, and participants from varied socio-economic backgrounds. Ability to remain calm and professional in high-stress situations and handle sensitive or confidential information with discretion and integrity. Ability to work independently and collaboratively while maintaining accurate program records and documentation. Proficiency in general office procedures and standard office equipment, including Microsoft Office applications. Ability to learn and apply County behavioral health programs, policies, and procedures. Required Qualifications A bachelor’s degree in a related field (social work, psychology, criminal justice, human development, counseling, or a related human service field). If the bachelor’s degree is in an unrelated field, there must be a minimum of two years of supervised clinical experience in a mental health setting. A minimum of one to two years of work or volunteer experience in behavioral health or social services Preferred Qualifications Prior completion of Mental Health First Aid (MHFA) Physical Attributes/Demands This position requires activities such as sitting, walking, standing, bending, stooping, climbing stairs and lifting light to medium objects, operating office equipment, and driving an automobile. Work Environment The work is performed indoors the majority of the time, in a hybrid office health care environment, in community facilities or in participant homes, and involves exposure to normal risks typical to either setting, including exposure to body fluid, cleaning chemicals, air borne diseases, emotionally labile patients and other hazards that may be present in private residential settings. Work hours are traditional business hours from 8:00 am to 4:30 pm; however, this position does require on-call responsibilities for emergency calls from program participants after-hours.
