We are seeking a dedicated and compassionate Physical Therapist to help deliver outstanding rehabilitation services and improve the quality of life for our residents.Your Role as a Physical Therapist (PT)At Solomons Nursing and Rehab Center, our Physical Therapist provides essential care by:Completing evaluations of residents for rehabilitation services in accordance with facility policiesAdministering treatment in compliance with physician prescriptions or referrals after reviewing patient history and progress reportsPerforming physical therapy procedures including therapeutic exercise, gait training, balance training, range of motion, and functional mobilityEstablishing realistic, patient-centered goals and progressing treatment plans appropriatelyWriting and charting evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge summariesReporting any significant changes in patient status to appropriate personnel (e.g., nursing staff or physician)Instructing residents, family members, and staff on treatment procedures, safety precautions, and home exercise programs when appropriateReporting equipment or supply needs to the Director of Rehabilitation and assisting with equipment carePerforming duties in an efficient, safe, and professional mannerRemaining flexible and adaptable to scheduling and facility needsPerforming other related duties as assignedWhat You NeedGraduate of an accredited Physical Therapy programValid Maryland Physical Therapist license or actively working toward licensureExperience in long-term care and/or geriatric settings preferred, but not requiredStrong communication, documentation, and teamwork skillsCommitment to high-quality, patient-centered careEEOCWe're an equal opportunity employer.
