ReLeaf - Is a transitional housing program providingtrauma-informed, no-cost housing and wraparound support services forsurvivors of domestic violence, sexual assault, human trafficking, andcommunity violence across Southern Illinois. This program fills acritical regional gap by offering safe, flexible housing, andindividualized case management to help survivors achieve long-termstability.[Program Coordinator j]{pasted="true"}ob duties include, but notlimited to:** Oversees daily operations of the PATH Outreach Program and ProjectReLeaf Transitional Housing Program, as directed. Will service as primary contact for ICJIA regarding programimplementation, compliance, reporting, and grant monitoringactivities. Coordinates referrals, intake screening, and program eligibility forindividuals served through PATH and ReLeaf. Intervenes and evaluates clients, establishes diagnosis, as needed.Assists in the development of the individual treatment plancollaboratively with clients and the QMHP by defining goals,specific outcomes and time frames for treatment. Monitor client\'sbehavior. Conducts crisis intervention to stabilize client\'s condition byutilizing short-term crisis intervention techniques and/or referralto other support services. Supports program staff in providing trauma-informed case management,housing stabilization services, and resource coordination, asneeded. Assists in ensuring clients receive individualized service planning,safety planning, and linkage to behavioral health, legal,employment, and community resources. Coordinates housing placement and stabilization for ReLeaf clientsutilizing communal, clustered, and scattered-site housing options. Assists in ensuring clients housing units meet safety, habitability,and program standards and coordinates with agency maintenance stafffor inspections and repairs. Maintains and develops collaborative relationships with internal andexternal partners including law enforcement, courts, victim serviceagencies, housing providers, advocacy organizations, and communitypartners. Ensures services are delivered in a trauma-informed, strengths-basedmanner consistent with victim rights, confidentiality standards, andagency policies. Provides direct services to program clients when necessary tomaintain service continuity; ensuring documentation is completedtimely and efficiently. Ensures transportation coordination and resource access for programclients when appropriate. Assists staff in identifying barriers to housing stability anddeveloping solutions to support long-term independence for assignedclients. Participates in local coalitions and community meetings addressinghomelessness, housing stability, and victim services, as applicable. Supports outreach efforts to ensure individuals experiencinghomelessness or victimization are aware of available services. Ensures program compliance with federal, state, and grant-specificrequirements including PATH and VOCA program standards. Oversees program data tracking, service documentation, and outcomereporting in the agencys electronic health r ecord system. Maintains accurate records and documentation to support compliance,audits, and quality improvement activities.Oversees the Arrowleaf Community Closet (Molina Closet) includingtracking and organization of items, spending, and MATCH, ifapplicable. Provides supervision to PATH outreach staff, the ReLeaf CaseManager, and the ReLeaf Residential Specialist.Monitors staff productivity, documentation accuracy, and servicerequirements.
