Full-Time Home Health Case Manager Registered NurseFull-time Home Health Case Manager Registered Nurse provides comprehensive, patient-centered care in the home setting. The role coordinates individualized care plans, collaborates with interdisciplinary teams, and ensures quality and regulatory compliance. It offers autonomy and the chance to improve patient outcomes.Key Responsibilities:Conduct comprehensive patient assessments and develop individualized care plansCoordinate and manage patient care across the continuum, ensuring quality and efficiencyProvide skilled nursing care in accordance with physician orders and established protocolsMonitor patient progress and adjust care plans as necessaryEducate patients and families on disease management, medications, and care techniquesCollaborate with physicians, therapists, social workers, and other healthcare professionalsMaintain accurate and timely documentation in compliance with regulatory standardsEnsure compliance with all state, federal, and agency guidelinesQualifications:Active Registered Nurse (RN) license in Idaho or compact licenseAssociate or Bachelor of Science in Nursing (ASN or BSN) requiredMinimum of 12 years of nursing experienceCurrent Basic Life Support (BLS) certificationValid driver's license and reliable transportationPreferred Skills:Home health or case management experience (preferred)Strong clinical assessment and critical thinking skillsExcellent communication and interpersonal abilitiesAbility to work independently and manage time effectivelyExperience with electronic medical records (EMR) systemsKnowledge of Medicare/Medicaid regulations in home health
