Healthcare Outreach Liaison

CareCore at Mary Scott is seeking an experienced and motivated Healthcare Outreach Liaison to help grow community partnerships and drive referral development for our skilled nursing and rehabilitation facility in Dayton, Ohio.This role is ideal for someone who enjoys relationship building, networking, and being out in the community while making a meaningful impact in post-acute care.The Healthcare Outreach Liaison will work closely with facility leadership and report directly to the Regional Director of Business Development.Responsibilities:Build and maintain strong referral relationships with hospitals, assisted living facilities, physicians, case managers, social workers, senior centers, and community organizationsConduct regular outreach visits throughout the Dayton marketDevelop new referral sources and community partnershipsMaintain communication with existing accounts to support continued referral growthRepresent the facility at networking events, community events, and healthcare meetingsCollaborate with admissions and clinical teams to support census growth goalsMaintain knowledge of skilled nursing services, payer sources, Medicare, Medicaid, and insurance processesTrack outreach activity and referral trendsQualifications:Previous healthcare marketing, outreach, admissions, or business development experience requiredSkilled nursing facility or long term care experience strongly preferredKnowledge of Medicare, Medicaid, managed care, and insurance processes requiredStrong communication and relationship-building skillsSelf-motivated with the ability to work independently and collaboratively as part of a team.Professional presentation and organizational skillsReliable transportation required for local travelWhat We Offer:Competitive salaryBonus/incentive potentialMedical, dental, and vision benefitsPTO and holiday pay401(k)Supportive leadership teamOpportunity for growth within a growing healthcare organizationIf you are passionate about healthcare relationships and helping connect patients and families with quality care, we would love to speak with you!
