The LVN will provide direct nursing care to patients in their homes, including vital sign monitoring, medication administration, and care coordination. Responsibilities also include working collaboratively with physicians, caregivers, and interdisciplinary team members to ensure personalized, high-quality care. The LVN will document patient progress, provide health education, and follow evidence-based practices for optimal outcomes.QualificationsCurrent and valid Licensed Practical Nurse (LPN) licensureExperience in Nursing and Medicine, with a deep understanding of clinical best practicesProficiency in providing Home Care and Patient Care, ensuring comfort and safetyExcellent communication and collaboration skills with a patient-first mindsetAbility to work independently while effectively coordinating with a multidisciplinary teamReliable transportation, valid driver’s license, and willingness to travel within Collin CountyCPR certification is preferredPrior experience in home health settings is a strong advantageCompensation - Salary or per-visit available, starting at 63k/annually and up based on experience
