Home Health Medical Social Worker (MSW)

Job Description We are seeking a Home Health Care Social Worker to join our team! You will provide emotional and administrative support to clients in their homes. You will work with a team of compassionate home health clinicians to drive positive patient outcomes.Responsibilities:Assess the nature of clients' situations by interviewing and reviewing their personal historyDevelop and execute individual treatment plansDetermine appropriate milestones to gauge client progressCommunicate client updates to all relevant partiesFacilitate referrals to other healthcare professionals and programsMaintain accurate client documentationQualifications:Previous experience in social work, counseling, or other related fieldsCompassionate and caring demeanorAbility to build rapport with clientsStrong leadership qualitiesExcellent written and verbal communication skillsCompany DescriptionDynamic Nursing Inc. is a privately owned Home Care Agency, specializing in client care since 1987.
