Job Title: Therapist - Community Outreach-Dale County
Reports To: Community Reinforcement Director
Summary:
A Community Outreach Therapist is responsible for ensuring that the comprehensive psychosocial needs of his/her clients are identified and effectively met in a safe and structured community treatment environment. A Community Outreach Therapist supervises a caseload of clients that have been referred to the DYS Community Reinforcement program by the local Juvenile Court. The therapist facilitates and implements programmatic/therapeutic interventions with clients in their homes, schools, jobs, and community. In addition, this position makes the necessary professional contacts in the community to ensure satisfactory continuation of services. This position works with Juvenile Probation Officers, Juvenile Courts, Community Representatives and Service Providers, and parents and/or legal guardians to ensure continuity of services and continuations of the program, assists with direct contact required for each client through the different phases to successfully complete the program.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
Manage caseload and documentation
Counsel individuals, groups or families to help them understand problems and develop treatment plans
Identify psychological, emotional or behavioral issues and diagnose disorders
Provide for referrals and community contacts
Qualifications:
Master's degree in Human Services field.
Have the ability to make social perception, to make good judgement, to have the ability to resolve complex problems, to monitor clients, and to have excellent decision making skills.
Have extensive experience in cognitive behavioral therapy and mind-body techniques.
Have excellent verbal and written communication skills up and down the organizational hierarchy.
Have experience working within a medical system and/or on a interdisciplinary team.
Have excellent computer skills
Have worked with Electronic Health Records/systems, be proficient and easily learn new systems of operations.
Must be able to work independently, without direct oversight and supervision.
