Their primary goal is to assess patient needs, develop personalized treatment plans, and facilitate individual or group therapy to prevent relapse and guide clients toward long-term recovery.Essential Job Functions:Interview patients, review records, and confer with other professionals to evaluate condition of patient.Formulate treatment plan for treatment and support of patient in a sober living environment.Provide counseling to patients individually and in group sessions to assist patients in their journey towards recovery.Counsel family members to assist family in dealing with and providing support for patients.Counsel family members individually or in group sessions regarding the impact of addiction in their lives. Participate in utilization review activities as required.Counsel patients/family members in crisis as needed.Maintain contact on regular basis with referral sources and community agencies.Coordinate and schedule referrals for program placement.Develop discharge/transition plans and completes discharge summaries timely.Refer patients to other support services as neededMonitor condition of patients to evaluate success and adapt treatment plans.Prepare and maintain reports and case histories.Formulate and conduct programs to promote prevention of alcohol and drug abuse.Understand and implement all TAP 21 Addiction Counseling Competencies.Attend weekly team meetings.Sign off and review Peer Mentor documentation.Document all notes timely, in Electronic Health Record.Communicate regularly with payer sources about authorizations for care.Maintain patient files and keep prepared for site visits.Comply with all CKF policies and procedures and related clinical licensure standardsOther duties as assigned.Physical Requirements:Based on an 8-hour daySit – Continuously – 5.5 – 8 hrs dailyStand - Occasionally - .25 – 2.5 hrs dailyLift/Carry 0-10 lbs. – Occasionally - .25 – 2.5 hrs dailyWalk – Occasionally - .25 – 2.5 hrs dailyReach Above - Occasionally - .25 – 2.5 hrs dailyBend/Stoop - Occasionally - .25 – 2.5 hrs dailyUse of hands – Pushing/Pulling - Occasionally - .25 – 2.5 hrs dailyUse of hands – Fine Manipulation - Continuously – 5.5 – 8 hrs dailyStress Level - MediumPHI Access Level:Level 2 – This position will have regular access to patient medical information, but no access to employee medical information.Work Environment:Office environment with moderate noise level.Benefits:Competitive benefits with partial employer paid contributions for health insurance and up to 4% for retirement plans.FLSA Classification:Salary ExemptDepartment:Bridgehouse SalinaSchedule:M-F; 8 am – 5 pm 40 hoursLocation / Category:On site; Salina Bridgehouse
