Consistently earning five-star CMS ratings, we lead with innovation, quality, and heart.Join us and make a difference every day-because here, caring is more than what we do, it's who we are.We are looking for an experienced, talented, and compassionate Hospice Social Worker for a full-time 40-Hour position in Hingham and Weymouth areas, MA.What will you be doing:The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through Croí Health. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges.
