If you're ready to grow with a company that's making a significant difference in the lives of others, you are made for this!Our Home Health Registered Nurse (RN) Case Managers enjoy some notable benefits:Visit rate is $70-75/visit or $150/ OASIS admission on Sat, Sun.Comprehensive Benefits: PTO, Medical, Dental, Vision, 401(K) with Company matchImpact You Can See: Shape care delivery that truly changes livesCommunity Connection: Be part of a locally owned agency deeply rooted in Lexington for over a decadeWork-Life Balance: Family-oriented culture that values your well-beingProfessional Growth: Online training, CEUs, and tuition discounts with Rasmussen University As a Home Health Registered Nurse (RN) Case Manager, here's a big picture view of what you'll do:Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standardsOASIS admissions and Coordinate with a team of LPNs, CNAs, aides and to ensure client and patient goals are metFollow the Home Health orders from PCPs working closely with their officeEnsure medical supplies/equipment are delivered and staff are trained on care proceduresAssess patient/client needs, resolve issues, and ensure a high level of satisfactionEffectively manage financial resources related to the delivery of quality patient/client care A few must-haves for a Home Health Registered Nurse (RN) Case Manager:Graduate of an accredited nursing program and active RN or BSN in MassachusettsMinimum of 1 year of nursing experience in home healthcare/ OASIS admissions, recerts, resumptions and DC'sAt least 1 year of supervisory and caregiver management experienceCPR certificationExtensive knowledge of state and federal home health laws and regulatory standardsExcellent management, organizational, problem-solving, communication and interpersonal skills, activities that may be required.
