Position Summary/Department Summary: MSW for Boston Children’s Hospital HRSN (Health Related Social Needs) Program. We are seeking a dynamic, social worker for Boston Children’s Hospital HRSN (Health Related Social Needs) Program. This is a unique opportunity to work as team member on a project with Department of Accountable Care & Integration (DACCI) addressing social determinants of health for primary care teens and young adults, and their families. This social worker will work with an interdisciplinary team at Martha Eliot Health Center (MEHC) and ACO staff to help ensure patient families have stable, healthy housing; access to nutrition resources; and receive all benefits and community resources available to them.Key Responsibilities:Identification and assessment of patients to determine eligibility for social support interventionsFacilitation of referrals to community partnersCase management and occasional short term crisis stabilization and support, including counseling and follow-up about accomplishing goals of plan and discharge from programCoordination of nutrition education and resources, including providing direct nutrition education and skills development (e.g., food budgeting) counseling and providing support to enrollment in community nutrition classesAssistance with program development and establishment of program infrastructureCollaboration and relationship development/management with partner community agencies, including co-management of patientsClinical and project documentation, data collection, and organization of meetings. LICSW supervision provided.Minimum QualificationsEducation:Master’s degree in social workExperience:Fluency in Spanish strongly preferred.Interest and ability in both macro and clinical social work and public health.Cognitive ability to observe, analyze and interpret general and technical data (verbal and written) accurately.Strong communication and interpersonal relationship skills with the ability to respond to events effectively.Interest in working with multicultural patients with complex needs.Ability to follow research protocols and procedures.Expertise with building community partnerships, trauma-informed care, social determinants of health, housing policy and resources, and food security ideal.Licensure: LCSW in the state of MA, LICSW in MA preferred.This position is located @ Martha Elliot Health Center in Jamaica Plain (free parking!)New hires are eligible for a $5,000 sign on bonus! The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
