Physical Therapist Location: Purcell, OK Agency: Cynet Health Pay: $2,306 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 5/25/2026 About the Position Job Title: Physical Therapist Profession: Physical Therapist (PT) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 1 year of PT experience; New Grads OK with Level II experience License: Oklahoma State License Must-Have: LTC or Rehab experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops an appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patients’ progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Participates in facility and company-required trainings, in-services, and conferences. Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support physical therapist assistants, rehabilitation aides, and/or students. Adheres to APTA and State Required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
