A company is looking for a Health Coach. Key Responsibilities Implement coaching techniques and behavior change solutions to improve patient outcomes Provide video consultations and support patient care plans using motivational interviewing and SMART goals Collaborate with interdisciplinary teams to ensure optimal patient-centered care and address any gaps in care Required Qualifications Board Certified Health and Wellness Coach 2+ years of experience as a health coach or wellness educator Proficiency in virtual care and database management Knowledge of motivational interviewing and behavioral change techniques University degree in health science or social science discipline (preferred)
