The ideal candidate is dependable, clinically confident, and enjoys working independently while still being supported by a strong clinical team.ResponsibilitiesProvide skilled nursing care under the supervision of an RN Case ManagerPerform wound care, medication administration, vitals, and disease process teachingAssist with care plan implementation and updatesConduct patient assessments and report changes in condition promptlyCollaborate with physicians, RNs, and other team members to ensure quality careMaintain timely, accurate documentation in the EMRTravel to patient homes within an assigned service areaQualifications Current Texas LVN license1+ year of nursing experience (Home Health preferred but not required)Strong assessment, communication, and clinical skillsReliable transportation, valid driver's license, and auto insuranceFamiliarity with EMR/mobile chartingCompassionate, dependable, and patient-focusedWhat We Offer Competitive pay + mileage reimbursementFlexible scheduling and autonomy in the fieldSupportive office staff and accessible clinical leadershipTraining and ongoing professional developmentOpportunities for growth into RN bridge programs or leadership roles
