Description ========= Job Summary: -Assesses and evaluates patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. -Based on this assessment and evaluation, develops a treatment plan, performs interventions aimed at improving and enhancing the patient's well being, and evaluates the patient's progress. -To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. -The requirements list must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.
