These services can be in an independent home, assisted living facility, or group home. The visits are made on a per visit basis. The home health aide is responsible for following the Aide Care Plan, observing patients, reporting these observations, and documenting observations and care performed.Essential Job Functions/ResponsibilitiesFollows the Aide Care Plan, initiated by the Registered Nurse, Physical Therapist or Occupational Therapist.Ability to provide competent hands-on personal care including: Sponge, tub and shower bath Bed bath Hair shampooing Nail and skin care Oral hygiene Toileting assistance Dressing and undressing Performs other duties as assigned by the Registered Nurse or Therapist, within the Certified or Licensed Nursing Assistant’s scope of practice.Position Qualifications Certified Nursing Assistant with current state certification or Licensed Nursing Assistant with current state license to practice in AZ.Home health experience is preferred. One year experience as a CNA/LNA is mandatory.Must have a valid driver’s license and able to work in the United States.Must have computer skills and ability to learn to use Electronic Medical Records Systems.Demonstrates excellent observation, problem solving, organizational, verbal, and written communication skills.Must be self-motivated and able to work independently.Current CPR certification.Current fingerprint clearance card.Reliable vehicle with current auto insurance.Good standing with the Department of Motor Vehicles.Must be able to pass a background check for pre-employment.Working Environment/Physical RequirementsProlonged or considerable walking, standing, and driving. Must be able to lift, position, or transfer patients and lift supplies/equipment. Considerable reaching, stooping, bending, and kneeling.
