SNI Companies is searching for LPNs to support our client in the Jacksonville, FL area. Our client is a vibrant, growing pediatric practice who is recognized as a national leader in pediatric population health and medical home integration. The ideal candidate will have previous pediatric patient care and case management experience. An active Florida LPN license is required. Contract-to-Hire Position Pay Rate: $27.00 - $35.00 an hour (depending on experience/education) Schedule: Monday through Friday, 8:00AM to 5:00PM (in-office) Licensed Practical Nurse (LPN) - Pediatric Clinic Position Summary: The Licensed Practical Nurse (LPN) in this pediatric clinic plays a vital role in delivering high-quality patient care through a combination of clinical support, case management, and telehealth services. This position is responsible for coordinating patient care, reviewing and communicating lab results, providing education to patients and families, ensuring compliance through medical record audits, and supporting clinic operations as needed. Responsibilities: Conduct telehealth appointments to assess patient needs, provide guidance, and support ongoing care plans under provider supervision Manage patient cases by coordinating care, tracking follow-ups, and ensuring continuity of care for pediatric patients Review laboratory results, identify abnormalities, and notify patients or guardians with appropriate instructions per provider direction Educate patients and families on treatment plans, medications, preventative care, and best health practices Perform regular audits of medical records to ensure accuracy, completeness, and compliance with clinic policies and regulatory standards Assist providers and clinical staff with patient overflow, including triaging patients, conducting newborn screenings, and documenting patient information Collaborate with physicians, nurses, and administrative staff to optimize patient care and clinic workflow Maintain accurate and timely documentation in the electronic health record (EHR) system Ensure patient confidentiality and compliance with HIPAA regulations Requirements: Active Licensed Practical Nurse (LPN) license in the state of Florida 3+ years of pediatric, outpatient, telehealth, and/or case management experience Knowledge of pediatric care practices and developmental stages Experience reviewing lab results and communicating clinical information to patients Ability to multitask, prioritize, and work efficiently in a fast-paced clinical environment Familiarity with quality assurance processes and medical record auditing Detail-oriented with strong organizational and documentation skills Strong communication and interpersonal skills, especially when working with children and families Ability to work both independently and as part of a collaborative care team Benefits: Medical, dental, and vision insurance plans Short and long-term disability plans Life insurance plans PTO and Holiday Pay Location: Jacksonville, FL
