Reintegration Mentor - Job #32Full-Time - $58,000 per yearWaterbury, CT 06702Mon, Tues, Thur, Fri 12p-8p and Wed 9a-5pABOUT THIS POSITION:The Reintegration Mentor will be a role model and will reinforce the adolescent's individual treatment plan as well as continued support for the adolescent and family after completion of the program. The family support aspect of the position is vital to assisting in creating and supporting a stable and secure home life for the adolescent to return to after the program (if applicable). Support could include, but is not limited to, assisting the family in acquiring basic needs such as housing, food, parental/familial mental health, and/or substance misuse treatment.Assist Clinician to assess needs and service options for clients and their familiesAssist Clinician with connecting youth to family facilitating meetings and visits as necessaryAssess appropriate services and linkages in the communityAssimilate and coordinate discharge of clients to evidence-based programs and other community servicesAdvocate for youth and their familiesAttend court or providers meetings as neededPlay a significant role in the client's treatment teamWork on flexible schedule, which includes week and evening hoursShare On-Call hours with other staffDevelop knowledge base of community resources across ConnecticutMaintain connection with youth and family throughout treatment and beyondPerform aftercare for REGIONS clientsBehave Ethically: Understand ethical behavior and business practices, and ensure that own behavior and the behavior of others is consistent with these standards and aligns with the values of the organizationFocus on Client Needs: Anticipate, understand, and respond to the needs of internal and external clients to meet or exceed their expectations within program parametersAbility to communicate, cooperate and serve people from all cultural identities with respect, professionalism, integrity and fairness in a manner that recognizes the individuality of each personFoster Teamwork: Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance program effectivenessRegular and consistent attendancePerforms other duties as assigned by supervisorSCHEDULE AND RATE OF PAY:35 hours per weekMonday, Tuesday 12:00pm-8:00pm, Wednesday 9:00am-5:00pm, Thursday and Friday 12:00pm-8:00pmHours may fluctuate to allow for later coverage to meet needs of clients and families in the community that can't be met in the morning.
