Primary Care Provider (NP, MD or DO) – Bronx, NYAbout the Role:Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? We are seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join our collaborative, multidisciplinary team.As a PCP, you key responsibilities will include:Clinical Care Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan. Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines. Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals. Document all care promptly in the electronic health record (EPIC). Provide primary care servicesCollaboration & Coordination Make appropriate internal and external referrals. Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.Quality & Professional Development Participate in quality improvement initiatives and case conferences.
