Community Services ProviderThe Community Services Provider is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to:Provide supervision and all direct care required to maintain safety and comfort of the person receiving servicesTransport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and explorationDevelopment and planning of inclusive community activitiesEstablish and maintain positive relationships with area businessesAssist people receiving services with completing daily or routine domestic or personal care tasksFoster and promote skills leading to independent livingAssist with the development of Habilitation Plans and following these plans for training and assistanceProvide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or requiredProvide supervision to people supported during program hours and during community outings as to ensure their health and safetyComplete required documentation in conformance with rules, regulations, policies and procedures, and billing practicesSPECIAL REQUIREMENTS:Possess good interpersonal, written, oral, and communication abilities and effective time management/organizational skillsBe computer literate with knowledge in agency operating systems and applicable software required for department needs.Have regular access to internetDemonstrate sound judgment in normal work and emergency situationsMust physically be able to perform required certifications and lift 50 pounds#humanservices #OPWDD #DSP #BinghamtonRequirementsHigh School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred.Possess a valid driver's license acceptable to the agency's insurance carrier
