Job Title: Registered Nurse - Home Health (13-Week Contract) Location: Waukesha, WI Contract Duration: 13 weeks Start Date: October 7th Guaranteed Hours Per Week: 40 Shift Requirements: 10-hour days, Monday - Thursday Travel Requirements: 6 patient visits per day, with each visit including drive time, patient interaction, and charting (approximately 1 hour per patient). Coverage across a 6-county service area, averaging around 250 miles per week (about 50 miles per day). On-Call/Call-Back Requirements: None Certifications Required (Attach Upon Submission): BLS (American Heart Association) WI RN License Valid Driver’s License OASIS Certification (Preferred) Minimum Experience Required: Current Home Health and Hospice experience (REQUIRED) Recent OASIS experience (REQUIRED) Charting Experience Required: Proficiency in Epic within the past 12 months Unit Information: Patient Ratio: 1:1 Patient Demographics: Adults 18 years and older Case Types: Chronic disease management (CHF, COPD, Diabetes, Cancer, CVA), wound care, IV administration Orientation: Day shift Candidate Qualities: Hard-working, strong time management skills, resilient, adaptable to change, and socially engaging. Dress Code: Any scrubs or corporate casual dress Additional Requirements: Homecare experience is essential. Candidates must be comfortable with potentially uncontrolled home environments (pets, smoke, etc.). Physical tolerance for driving, navigating stairs, and handling uneven landscapes is required. Visits must occur regardless of weather conditions.
