This role is ideal for someone who thrives in a collaborative environment and is passionate about improving outcomes for complex and underserved patient populations.Unit OverviewHospital-wide role (unit sizes vary)Assignment to a primary unit with potential to float across inpatient unitsWorks closely with interdisciplinary teams to ensure safe and timely patient transitionsPatient PopulationPrimarily adult and geriatric patientsComplex cases including:Unhoused and low-income populationsPatients with limited access to resourcesMay support units such as:Med/SurgPCULDRP(ICU coverage possible but less common)CaseloadTypically 14-20 patientsRequired QualificationsMinimum 3 years of acute care case management experienceFirst-time travelers: AcceptedEducation & CertificationsASN or higher requiredNo specific certifications requiredRequired SkillsStrong care coordination and discharge planning experienceAbility to:Collaborate effectively with physicians and multidisciplinary teamsNavigate insurance plans and authorization processesIdentify and coordinate community resourcesExcellent:Assessment and critical thinking skillsCommunication and interpersonal skillsMust be a team player with a proactive approachSupport & ResourcesDepartment leadershipTeam of Case ManagersCase Management AssistantsHospitalist coverage 24/7Technology & EquipmentEMR: EPICCare Coordination Platform: Careport (for referrals and placement)FloatingAssigned to a primary unit but may float across inpatient units as neededOrientationNew Employee Orientation (NEO)3 shifts of department orientation (with possible extension if needed)SchedulingShift: Monday-Friday, 8:00 AM - 4:30 PMOccasional late days as neededWeekend Rotation: Every 5th weekendIf working weekend, receives following Monday & Friday offOn-Call: Not requiredHoliday Requirement: Approximately 50% (up to 2 holidays per contract)Schedule Type: Set schedule with weekend rotationAdditional InformationDress Code:Eggplant-colored scrubs or business casualWhite lab coat requiredParking: Free on-site parkingWork Environment:Collaborative, mission-driven teamFocus on complex care coordination and patient advocacyIdeal for strong communicators with a passion for serving diverse populations
