Were currently seeking a Licensed Practical Nurse (LPN) to join our growing team for on-call and per diem home visits.What Youll Do:Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.Educate clients and families on health conditions and self-care.Document all care activities in accordance with agency protocols and EMR requirements.Respond to client needs during assigned on-call shifts and per diem requests with professionalism and empathy.Collaborate with interdisciplinary teams to ensure continuity and quality of care.Who You Are: Licensed Practical Nurse (LPN) in the state of Connecticut 1+ year of nursing experience (home health preferred) Strong communication, critical thinking, and documentation skillsComfortable working independently in home settingsReliable transportation, valid drivers license, and auto insuranceTech-savvy and familiar with EMR systems (training provided)Why Join Us?
