Key Highlights - Weekly pay: $2,377-$2,443 per week - Contract duration: 13 weeks - Shift: Days, 8 hours - Travel assignment in a home healthcare - hospice setting - Start date: 05/04/2026 - Represented by RCHEA Responsibilities - Administer APTA-standard physical therapy treatments for disabilities, injuries, and diseases to relieve pain, restore motor function, and maintain optimal performance. - Assess patients, develop and implement individualized PT plans, and monitor progress toward goals. - Educate patients and caregivers on exercises, positioning, and strategies to maximize safety and independence at home. - Document evaluation findings, treatment plans, progress notes, and discharge planning with clear communication to the care team. - Maintain equipment, ensure patient and staff safety, and contribute to overall quality of care in the home-based hospice setting.
