DescriptionOccupational Therapist (Health Programs)Reports to: Director of WellnessSUMMARY:The Occupational Therapist (OT) will work in coordination with clinic staff to help patients improve their ability to perform tasks in their daily living and working environment.MINIMUM QUALIFICATIONS:Master's DegreeOccupational therapist licensure5 years of experience.ESSENTIAL DUTIES:Conduct patient assessments and evaluations to determine the patient's physical condition, limitations, medical history and personal goals.Communicate with clinic staff to confirm plans of care.Advise patient's family members and other supportive individuals regarding the level of care for the patient.Provide OT guidance, support and education during patient sessions.Instruct patient how to interact with adaptive technology and how to use assistive devices.Guide patient through graded activities.Generate documentation for patient and patient filesOther duties as assigned
