The PTA helps implement individualized therapy plans designed to restore function, prevent disability, and promote wellness for older adults living in the community.Essential Job Functions:Deliver physical therapy interventions per the established plan of care, under the direction of the Physical Therapist.Observe participant response to treatment and report findings to the supervising therapist.Document daily treatments and participant progress in the medical record system.Collaborate with the interdisciplinary team (IDT) to coordinate participant care.Assist with participant transfers, mobility, and the use of adaptive equipment.Monitor equipment maintenance and report safety concerns.Plan individual and group activities and implement treatment according to plan.Educate and instruct participants and family members in proper care and use of various supports and exercise programs and in use of durable medical equipment in the home.Exhibit communication skills necessary to interpret age specific responses to treatment.Train PACE staff in methods to maximize safety and functional abilities.Know and adhere to the philosophy and goals of Bluestem PACE.Transport participants and/or equipment in one's personal vehicle or PACE vehicle as needed.QualificationsQualifications and Experience:Must be at least eighteen [18]-years of ageMust have working knowledge of Microsoft Office SuiteMust have graduated from an accredited Physical Therapist Assistant programMust have a current Kansas PTA license in good standingMust have CPR certificationMust have a valid driver's license and reliable transportationPrefer experience with older adults or in a geriatric settingPhysical Requirements:Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
