Registered Nurse Case ManagerThe Registered Nurse Case Manager is responsible for delivering high-quality, patient-centered hospice care while managing a caseload of patients across designated counties. This role focuses on direct patient care, interdisciplinary collaboration, care coordination, and documentation to ensure compassionate end-of-life support for patients and families.Schedule & Work Details:Weekly Hour Guarantee: 36 hoursSchedule: Monday through Friday, 8:00 AM to 4:30 PMOn Call Requirements: On call 23 times per week and every 3rd weekend (Saturday and Sunday)Coverage Area: Mahoning, Columbiana, Trumbull, and portions of Portage counties including Garrettsville, Windham, Deerfield, Atwater, Nelson, Palmyra, and ParisMileage Reimbursement: Yes, reimbursed between patient visits from first to last patientPay Rate: $51$53 per hourKey Responsibilities:Provide direct hospice patient care, including medication management, wound care, Foley catheter placement, and ongoing assessmentDevelop, implement, and update individualized care plans in collaboration with physicians and the interdisciplinary care teamComplete timely and accurate clinical documentation and charting while meeting productivity expectationsParticipate in on-call coverage, weekend rotations, weekly case conferences, and team meetingsMaintain frequent daily communication with clinicians and office staff to ensure coordinated and seamless patient careRequired Qualifications:Active Registered Nurse (RN) licenseHospice or home health nursing experienceAbility to manage an independent caseload and meet productivity standardsAll clinical skills up to datePreferred Qualifications:Experience using Homecare Homebase charting systemPride Global and its affiliates offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment Opportunity Pride Global and its affiliates are an equal opportunity employer.
