Home Health Nursing Manager
Description:We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes.DutiesConduct comprehensive assessments of patients' health status and needs in their home environment.Develop, implement, and evaluate personalized care plans based on patient assessments.Administer medications and treatments as prescribed, ensuring adherence to safety protocols.Provide education to patients and families regarding health management, medication administration, and care procedures.Monitor patient progress and adjust care plans as necessary in collaboration with the healthcare team.Supervise home health aides and support staff in delivering quality care.Maintain accurate documentation of patient care activities, assessments, and outcomes.Collaborate with social workers to address social determinants of health affecting patient well-being.Utilize financial acumen to assist patients with navigating healthcare resources and budgeting for care needs.RequirementsCurrent Registered Nurse (RN) license in the state of practice.Experience in home community care or long-term care settings such as nursing homes or assisted living facilities is preferred.Strong knowledge of medication administration practices and safety protocols.Excellent interpersonal skills with the ability to communicate effectively with patients, families, and interdisciplinary teams.Familiarity with social work principles to support holistic patient care.Ability to supervise support staff while fostering a collaborative team environment.Strong organizational skills with attention to detail for accurate documentation and reporting.Commitment to providing compassionate care that respects the dignity of each patient.Join us in making a difference in the lives of our patients by providing exceptional home health nursing services.
