As a Speech Therapist, you will provide skilled speech therapy services by assessing, planning, and implementing individualized therapy programs to help improve speech, language, and swallowing abilities. You'll work with patients to restore their communication skills and assist with eating and swallowing difficulties, all while providing compassionate, personalized care. Key Responsibilities: Conduct comprehensive speech, language, and swallowing assessments for patients in home health settingsDevelop and implement individualized therapy plans tailored to the specific needs of each patientProvide direct speech therapy treatment, including exercises for speech, language, and swallowing disordersCollaborate with interdisciplinary teams to ensure a holistic approach to careEducate patients, families, and caregivers on strategies for improving communication and swallowing abilitiesDocument therapy sessions and progress notes according to home health policy and regulatory standardsMonitor and track patient progress, adjusting treatment plans as necessaryEnsure all therapy activities are compliant with state and federal regulationsMust Have: Valid state Speech Therapist license (required)Strong knowledge of speech therapy techniques for various conditions, including swallowing and communication disordersExperience in a Home Health setting (preferred but not required)Excellent communication and interpersonal skillsAbility to work independently and as part of an interdisciplinary teamCompassionate, patient-focused approach to careNice to Have: Experience with home health therapy servicesFamiliarity with Kantime EMR other electronic health record systemsCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) Equal Opportunity Employer We celebrate diversity and are committed to providing equal employment opportunities to all qualified applicants, regardless of race, color, religion, sex, gender identity, sexual orientation, age, national origin, disability, or veteran status.
