The LPN/LVN demonstrates active collaboration, communication and problem solving with the interdisciplinary and Care Team (Patient, Licensed Independent Provider, Pharmacist, RN, and Medical Support Assistant) and Expanded Care Team (family/caregiver, internal and community-based services involved in providing care to the patient).Major Duties and Responsibilities:Patient Care: This element demonstrates the direct care provided for the veteran patient. The LPN/LVN will demonstrate nursing specific skills within the scope of an LPN/LVN. Examples include but are not limited to:a. Technically proficient in initiating, performing, and completing assigned duties in providingcare to variable patient populations.b. Utilizing computer programs required for the position.c. Carries out assigned patient care based on the patients' conditions; to use judgment inselecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevantpatient information.d. Observes, identifies and responds to the patient's needs for medication, equipment-assisted care, and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.e. Utilizing principles of aseptic technique, universal precautions, and isolations requirements.f. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.g. Performs support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing, and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure.h. Assist with and performing activities of daily living, i.e., bathing, toileting, feeding, shavings, oral hygiene, care of scalp and nails.i. Taking and recording in the medical record: vital signs, blood pressure, pulse, respiration, pulse oximeter, temperature, pain score, weight, and neurological check.j. Collecting specimens, urine, feces, sputum, etc., and appropriately prepare and transports specimens to the laboratory. Measuring and recording total intake (oral, IV, per tube) and output (urine, emesis, wound, etc.).k. Monitoring patients receiving blood products.l.
